An Adult Development Home (ADH) is a licensed residential setting operating under a foster care license. In an ADH, an adult with a developmental disability lives in the care provider’s private home and receives individualized, 24-hour support.
Provide daily supervision and support to ensure the member’s safety, well-being, and comfort in a home-based setting.
Administer and monitor medications in accordance with medical orders and ensure proper documentation and compliance.
Assist with scheduling, transportation, and attendance at medical and healthcare appointments as needed.
Facilitate transportation to appointments, activities, and community outings to support independence and engagement.
Support activities of daily living based on the member’s Person-Centered Service Plan (PCSP), promoting independence whenever possible.
Complete and document habilitation (HBA) goals to support skill development, independence, and individualized progress.
ADH providers work in partnership with the agency and the member’s care team to support long-term stability, independence, and quality of life.
