CommunityCares is a free screening tool made available through the Arizona Health Care Cost Containment System (AHCCCS). It is designed to help identify and address Health-Related Social Needs (HRSN) that may impact overall well-being. 

The screener helps identify potential gaps in areas such as: 

• Housing instability or homelessness 

• Food insecurity 

• Transportation needs 

• Employment instability 

• Utility assistance 

• Interpersonal safety 

• Justice or legal involvement 

• Social isolation or limited social support
